And he's nervous. He knows you love him and he loves u too, but he's wondering if you'll still like him after the surgery, he wants to think it's a dumb question and that u will but doubt and anxiety fill his mind. I made my character (Named him “Iakin” (Ee-auh-kin/Ee-ah-kin) like- two feet tal, er than Kai or something. I described it as “Iakin is so much taller than Kai, that Kai’s arms wrap around Iakin’s lower waist and his face presses against his mid-back.” (Kai was hugging Iakin from behind)
