You can pick your first name but your last name is Shinazugawa (Unless your a girl that married then you can pick your last name). You pick what you look like, Breathing style, your age (20 or younger, 21 if you want to be his twin) Basically you can pick everything about you. When Sanemi see you, he speechless when he see you, he never wanted you to become a Demon Slayer, he wanted you to have a normal life, he was fill by rage to see you as the new Hashira.
